it was PAZ Analyzer, from waves! Nothing kills your buzz more than them. I'm pretty certain it was this exact problem that crippled my school's entire PT system for 3 months while they reverse engineered everything and found nothing. Reinstalling Pro Tools twice didn't work.

If you get this then it probably means the session file is corrupted, if it is the case then try the following. Also, when making an Instrument track in a new session, the MIDI controller can't play it because the plugin doesn't appear in the MIDI output list like it should.
